What we collect.
When you request an appointment or a benefits check, we collect what you type into those forms: your name, phone number, email address, what you tell us about your situation, and, for benefits verification, your date of birth, ZIP code, insurance provider, member ID, and group number. If you use the interactive tools on our homepage, your selections may be stored briefly in your browser and submitted anonymously so we can understand what visitors are looking for. We do not run third-party advertising trackers on this site.
Why we collect it.
To call you back, to schedule your visit, to verify your insurance benefits before your first appointment, and to communicate about your care. If you check the text-message consent box, we use your phone number to send appointment-related texts. Message and data rates may apply, and you can reply STOP at any time to opt out.
What we never do.
We never sell your information. We never share it with advertisers. Insurance details you submit for a benefits check are used only to verify your coverage and are handled by our scheduling team and no one else.
Service providers.
Form submissions are processed by our website hosting provider and delivered to our practice-management systems so we can respond to you. Live review content is displayed from Google via an embedded widget; interacting with it is subject to Google’s own privacy terms. The map on our contact section is embedded from Google Maps. We use Google Analytics and Microsoft Clarity to understand, in aggregate, how visitors use this site, and our text-chat window is provided by LeadConnector; each of these services may set cookies and is subject to its own privacy terms.
Your health information.
Once you become a patient, your health information is protected under HIPAA and our Notice of Privacy Practices, which you will receive at your first visit. This website is not intended for submitting detailed medical records; please save the full history for your evaluation, where it belongs.
Retention and access.
We keep form submissions as long as needed to respond to you and to meet our legal obligations. You may ask us what we have on file for you, ask us to correct it, or ask us to delete it where the law allows. Call (864) 263-7390 or email hello@zonept.com and a human will handle it.
